Consultation Information: What to bring to your appointment
There are several things you may need to bring on the day of your appointment. These items will help us with your care and plan your surgery better.
Referral letter from your general practitioner (GP) or family doctor. You may make direct appointments with us and come without a referral letter.
Copies of any imaging and test results you have such as X-RAY films, MRIs, CT scans, and blood tests.
Your private health insurance information
Medicare card, Department of Veteran Affairs card (DVA), Pension card
So we have a complete patient record, you may need to provide some personal information and answer a questionnaire about your health prior to your consultation. You may need up to 10 minutes to complete this questionnaire but our friendly staff will be available to assist you.
Our specialist usually runs on time and according to schedule but in some cases you may experience a short delay. We do appreciate your time is precious but please excuse this delay. Sometimes, Dr Roderick Kuo may need to spend extra time to review a patient’s condition, plan a complex surgical procedure, or thoroughly explain information to a patient.
If you feeling very unwell or uncomfortable, please let our staff know immediately.
The items listed above are not compulsory for a consultation with Dr Roderick Kuo. Thanks to operating clinics in established locations, imaging services such as X-ray and CT scans are readily available onsite and our staff members can assist you with matters concerning your private health insurance.
